| The renal failure module facilitates the monitoring of patients with renal failure. The modual covers the specific monitoring requirements of patients in the following catagories
Patients with renal failure will fall into two broad categories. Those who have chronic renal disorders and are generally stable and those who are unstable and/or require more detailed investigation. The former category of patients are frequently managed by a shared care scheme where the patient visits their GP for routine checks to be performed and blood and urine samples taken for analysis. The results of the analyses and the GP's clinical observations are then reviewed by a hospital renal specialist and an appropriate revised management plan is established. The latter category of patients require more varied management within a secondary-care environment. The HICSS renal failure modual provides the data capture, information retrieval and clinical decision support for all of the patient scenarios listed above. The modual also efficiently handles the issues that arise from the shared care scheme where two geographically separated clinicians are involved in the patient management process. This is achieved by generating pro-formas and request forms for the GP to complete and a work flow process to ensure that information is recieved and processed within an acceptable time period. The renal modual utilises pathology result data acquired via an interface to the hospital pathology system and may be supplemented with pathology results obtained from other laboratories. Pathology results are presented in a variety of profiles to suit the specific category of renal failure for each patient. Cockcroft and Gault creatinine clearance is calculated automatically by the module and an End Stage Renal Failure (ESRF)date predicted where C&GCC is trending downwards. Other calculated ratios and indices may also be easily and quickly viewed by the renal specialists. |
